Step 1: Open Enrollment Information
- If you are a resident of Fall Creek School District, Skip Step 1
- If you do NOT reside in the Fall Creek School District, you MUST apply for open enrollment and be accepted prior to completing the new student registration form.
- If you are unsure if you reside in the Fall Creek School District, please contact teresareetz@fallcreek.k12.wi.us
Step 2: Complete New Student Registration
- If you already have students enrolled in the District, please log in to Skyward, and complete the new student enrollment. Located below the home button on the left navigation menu.
- If you are new to the district click on the new student registration link. This link creates a username and password for Skyward access. New Student Registration
- Once you receive your confirmation email from Skyward, continue your enrollment application(there will be a link in the email). New student enrollment is located below the home button on the left navigation menu.
- Student enrollment contains the following information
- Student Information
- Family/Guardian Information
- Medical/Dental Information
- Emergency Contact Information
- Additional District Forms
- Once completed, (Submit the Application to the District)
Step 3: Required Documents to Complete Enrollment
- Proof of student's age: Present original Birth Certificate to the building secretary.
- Proof of Residency (one of the following): Current Lease Agreement containing name, address, effective date, landlord's name, and signatures, or current utility bill (gas and electric) containing name and address
- Immunization form (required if vaccinations were not given in Wisconsin)
- Other important documents if applicable (IEP, court documents, etc.)
